The Recap, In My BodyA recapitulação no meu corpo

Chapter 1Capítulo 1

The Five Layers, Now With an AddressAs cinco camadas, agora com endereço

Almost everyone learned in school that sex is a switch with two positions: XX or XY, on or off. Biology doesn't work that way. Sex is a stack of layers, and each layer can follow its own path inside the same body.

In Part 1 I separated five: the chromosome, the hormone, the receptor that reads the hormone, the gonad, and the anatomy. When all five point the same way, nobody notices there are five. When one of them steps out of line, the little "this or that" box stops closing.

In Part 1 I explained the layers in the abstract, with examples from other conditions. Here I'll do it differently. I'll run my own body through all five, one by one, and show the mechanism behind each along the way.

Chromosome: XY. The Y chromosome carries a gene called , for sex-determining region Y. It's usually the first domino: it pushes the embryo's gonad toward becoming a testis, and the rest of the cascade follows. Usually. If sex were only the chromosome, this conversation would end here, and my body would be read as male to the very end. It doesn't end here. The chromosome is the starting point, and in my case it was the first one left behind.

Cromossomo: XY.

Hormone: high estradiol. Testosterone and estradiol are not hormones on opposing teams. They're chemical relatives, and one turns into the other. There's an enzyme, , whose whole job is converting testosterone into estradiol. Every body has aromatase; the question is how hard it works. Before any medication, my labs already showed high estradiol, sustained for months, and falling testosterone. In my body, aromatase converts almost everything. The hormone the textbook says I should have turns into the other one before it gets where it was going.

No meu corpo a aromatase converte quase tudo.

Receptor: . A hormone on its own does nothing. It needs a receptor, a kind of lock inside the cell: the hormone fits in, the receptor changes shape, moves into the nucleus and switches on the genes that do the work. PAIS stands for partial androgen insensitivity syndrome, and it means the testosterone lock opens badly. The signal arrives, but only part of it gets read. And "partial" isn't a point, it's a range. Medicine uses scales, like the Quigley scale, that run from bodies that are almost typically male, where a single detail gives the insensitivity away, to bodies that are almost typically female. Two people with PAIS can have very different bodies and the same acronym on their chart. In Part 1, when I explained PAIS, I compared it to a radio that only half picks up the station. Today that radio is mine.

Receptor: .

Gonad: . The gonad is the organ that produces gametes and sex hormones. In a typical body it becomes an ovary or a testis, and the choice happens early, still in the embryo. Mine didn't choose. It's an ovotestis, with tissue of both kinds in the same organ. And alongside it there's a structure medicine calls , which is where the uterus comes from. That deserves a whole chapter, and it gets the next one.

Anatomy: the organ I have. Every embryo's external genitalia start out the same, from the same tissue. What decides whether it develops into a clitoris or a penis is the androgen signal, read by that same receptor. With the receptor reading only half, the tissue gets fewer orders to masculinize. In my case, as I describe it, the result is an organ far closer to a clitoris than to anything else: small, positioned like a clitoris, and working like one.

Five layers. Five answers that don't match each other in the manual. One body.

Cinco camadas.

Fig. 1Five layers, before and afterCinco camadas, antes e depoisSwitch between November 2025 and October 2026 and see which layers changed and which stayed.

The same five-layer diagram from Part 1, with the author's body on two dates. Two changed: the hormone, which stopped coming only from inside and now includes estradiol gel and cyproterone, and the anatomy, which kept going in the direction it had already chosen. Three stayed: the XY chromosome, the PAIS receptor and the gonad, an ovotestis, which is next to change, in January.

And this is where "before and after" gets interesting, because not everything moved. The chromosome didn't change, and won't. Neither did the receptor. What changed since November is the hormone, which stopped coming only from inside, and the anatomy, which kept going in the direction it had already chosen. The gonad is next to change, and the one changing it will be me, in January. But that's for Block IV.

Notice one thing. Of the five layers, only one was visible on the day I was born, and it was the very one that depended on all the others. Anatomy is the result, not the cause. Reading the sex of a body from its external anatomy is reading the last page of a book and swearing you know the plot.

Repara numa coisa.

Now, the objection that always comes, because this is a site that presents itself as science before opinion: "this is one case, and one case proves nothing."

I agree with the first half.

Concordo com a primeira metade.

I'm not using my body as a rule for anyone. I'm using it as a microscope slide. Nobody looks at a slide thinking every cell in the world is that one. You look to see, in a real cell, a mechanism that applies to all of them. What's general here isn't my combination, it's what lies underneath it. Aromatase, which converts one hormone into the other; the receptor, which can read a signal by half; the signal that erases a uterus or lets it stay, which is the subject of the next chapter; the tug of war between the pathways that decide whether a gonad becomes an ovary or a testis, which is Block II. All of that is in endocrinology textbooks. The only thing that's mine alone is the order in which those pieces fell.

The mechanism belongs to everyone. The combination is mine.

O mecanismo é de todo mundo. A combinação é minha.

Chapter 2Capítulo 2

The Uterus Nobody SawO útero que ninguém viu

Nobody saw my uterus at birth.

Ninguém viu o meu útero no nascimento.

That's not a figure of speech. When I was born, someone looked at the outside of my body, decided what they were seeing, and wrote the decision on a piece of paper. Nobody looked inside. Nobody had a reason to look, because the outside seemed to answer the question.

Today I know there's a uterus inside my body, and ovarian tissue producing estrogen. The pain from the prologue comes from there. I menstruate. And menstruating isn't a detail: it's proof that the organ isn't some forgotten leftover. It's an organ that responds, that has a cycle, that works.

A dor do prólogo sai dali. Eu menstruo.

That uterus didn't show up in 2025. It was there before I was born.

Esse útero não apareceu em 2025. Ele estava lá antes de eu nascer.

What showed up in 2025 was someone looking.

O que apareceu em 2025 foi alguém olhando.

To understand how a uterus survives in an XY body, you have to go back to fetal life. At the start, every embryo has two structures side by side, and each is a draft. One can become the internal male reproductive system; the other, called after the German anatomist Johannes Müller, who described it in the nineteenth century, can become the fallopian tubes, the uterus and the upper part of the vagina. One of the two drafts has to be erased.

The draft gets erased by a hormone called , anti-Müllerian hormone. It's made by Sertoli cells, inside fetal testicular tissue, right after the testis starts to form. And two details change everything.

The first: AMH acts locally. It doesn't spread through the whole body like a general broadcast. It works up close, and erases the Müllerian structure on the same side as the gonad that makes it. In animal models with an ovary on one side and a testis on the other, the tube and uterus appear only on the ovary's side. On the testis's side, nothing is left.

O primeiro: o AMH age localmente.

The second: the Müllerian structure only obeys AMH during a short window. If the signal arrives weak, late, or on one side only, the window closes, and whatever wasn't erased stays. After that, no amount of AMH helps: the structure stops listening.

Now put both details together with what an is. The testicular tissue inside an ovotestis is often malformed, and the signal that should erase the Müllerian draft comes out incomplete. That's why, in people with , what's found inside varies enormously: remnants of both structures, half a uterus, or a whole one. The medical literature records every version.

A literatura médica registra todas essas versões.

My uterus stayed because the signal that should have erased it didn't arrive strong enough, in the right place, in time. Nobody chose that. Not me, not my parents, not any doctor. It was decided before I had a name.

The body decided first. Literally.

O corpo decidiu primeiro. Literalmente.

Think about what that does to the phrase "sex assigned at birth." Assigned based on what? On the only layer you could see without opening anything: external anatomy. One layer out of five. The chromosome, nobody read. The receptor, nobody tested. The gonad, nobody knew what it was. And the uterus, nobody saw.

They read the cover and catalogued the book.

Leram a capa e catalogaram o livro.

This isn't an accusation against whoever registered me. Nobody examines the inside of a baby that seems to answer the question from the outside. The problem isn't the look, it's the certainty that came after it: that the outside had said everything, and that the paper could be filled in with one of two boxes without fear of being wrong. That certainty is what turns a reasonable guess into a document, and a document into a destiny.

I spent twenty-five years with a uterus no paper knew existed. When the inside finally showed up, it didn't agree with the box that had been checked. Much of the rest of this text is about what happens after that: to the body, to the paper, to the doctors, and to me.

Medicine looked at the outside and stopped. My body kept going.

A medicina olhou pra fora e parou. O meu corpo continuou.

Chapter 3Capítulo 3

Tug of WarCabo de guerra

In school, the story of the gonad is told as an order from the top down. The Y chromosome has a gene, ; SRY gives the order, the gonad obeys and becomes a testis. No Y, no order, and the gonad becomes an ovary for lack of instructions. One active pathway and one lazy one.

That story is wrong at both ends.

Essa história está errada nas duas pontas.

The embryo's gonad starts out bipotential. It has, at the same time, the equipment to become a testis and the equipment to become an ovary, and both sides are switched on from the start. On one side, SOX9 and FGF9 pull: they push the gonad toward testis and reinforce each other in a loop that feeds itself. On the other side, RSPO1, WNT4 and FOXL2 pull toward ovary. And here's the detail that changes everything: each side doesn't just pull for itself, it holds the other side down. WNT4 restrains SOX9 and FGF9. FGF9 restrains WNT4.

A gônada do embrião começa bipotencial. O WNT4 segura o SOX9 e o FGF9. O FGF9 segura o WNT4.

It isn't an order. It's a tug of war.

Não é uma ordem. É um cabo de guerra.

SRY's role in this story is like someone who joins the game and gives one side a hard pull at the right moment. If the pull comes strong and on time, the testis side gains momentum, the SOX9 loop locks in, the ovary side goes quiet, and the whole rope goes one way.

And "on time" is literal. In a human XY embryo, the gonad stays bipotential until about forty days after conception, when SRY switches on. In mice, Hiramatsu and colleagues measured the window in which SRY can still turn the game around: about six hours. And what closes that window is the balance between FGF9 and WNT4 itself, the two sides of the rope. After that, the pull comes too late.

The proof that the ovary isn't "a lack of instructions" came from mouse experiments: remove WNT4 from an XX gonad, and SOX9 and FGF9 rise on their own, with no SRY at all. The ovary side had been actively holding the testis side down the whole time. Leo DiNapoli and Blanche Capel called it by its name: a standoff. The fate of the gonad is contested by both sides.

O lado do ovário estava segurando ativamente o lado do testículo o tempo todo.

So imagine what happens when neither pull is strong enough. When the push toward testis comes weak, late, or only in part of the tissue. The rope goes nowhere. Part of the gonad becomes testis, part becomes ovary, and the two live together in the same organ.

That has a name. It's an .

Isso tem nome. É um .

Fig. 2Tug of warCabo de guerraDrag the knot to either mark, or hold ← or →.Arrasta o nó até uma das marcas, ou segura ← ou →.

Both pathways pull at the same time: SOX9 and FGF9 toward testis, RSPO1, WNT4 and FOXL2 toward ovary, and each side holds the other down. You pull. The rope barely moves, your strength runs out and it slides back to the middle. Nobody wins: the result is a draw, an ovotestis.

Notice what the interactive teaches you with your hand: you can pull as hard as you want and the rope won't go. It's not that you lack strength. It's that winning never depended on whoever was holding the rope from the outside.

Nobody chooses how the gonad turns out. Not the embryo, not the parents, not a doctor. It's a game decided inside the tissue, in a matter of weeks, before there's a person to have an opinion about it.

And my case falls into a rare corner of an already rare picture. Among people with , most have an XX karyotype. Some are mosaics, with cells of different makeups in the same body. XY people, like me, are the smallest slice: depending on the series studied, somewhere between 7 and 15 percent of cases. Which means my body had a Y, and the Y didn't win the tug of war.

There's one last thing biology discovered about this rope, and it's the one that moves me most. For a long time people thought the tug of war ended in fetal life: gonad decided, decided forever. In 2009, Mathias Treier's group showed in adult female mice that switching off a single gene, FOXL2, was enough for an adult female's ovary to start turning into a testis, with cells changing identity and testosterone rising to male levels. Two years later, David Zarkower's group showed the mirror image: switching off the DMRT1 gene in the adult testis of mice made the cells start reprogramming in the female direction, with switched on and estradiol rising.

Both sides keep holding each other back for a whole lifetime. The authors' own conclusion is that keeping an ovary an ovary is an active process, one that lasts for life.

I won't say that's what happened to me, because I don't know, and because a mouse isn't a person. What I'm saying is more modest and stronger: the idea that a gonad's sex is decided once and sealed like an envelope is no longer what biology says. The rope stays taut. It only looks still because, in most bodies, one side won by a wide margin.

A corda continua esticada.

In mine, nobody won. The ovotestis is a draw, and the draw was settled before me.

Chapter 4Capítulo 4

The Y That Gave Me No StrengthO Y que não me deu força nenhuma

If you want to know which side I'd land on in a test that looks for the Y chromosome, the answer is simple: the male side. My karyotype is XY. A test that only looked at that would put me in the men's line and end the conversation.

Today I can't carry a checked suitcase.

I mean that literally. A regular airport suitcase, the kind everyone hauls to the conveyor belt without thinking, I can't carry anymore. And after intense days, what's left isn't mental fatigue: it's muscle pain and a tired body.

Those two sentences live in the same body, and that's why they're worth a chapter. The Y in my karyotype didn't turn into strength. And understanding why means understanding that chromosome isn't destiny even in muscle, which is where everyone assumes it's in charge.

The chain that turns a Y into muscle has several links, and each one has to work. The Y usually starts the formation of a testis. The testis produces testosterone. Testosterone travels through the blood to the muscle. And there, in the muscle, it has to fit into the , that lock from chapter 1, for the cell to get the order to grow. The body composition literature describes the androgen receptor as central to the differences in muscle mass between the sexes, and experiments in mice that remove that receptor only from muscle change lean mass.

Now put my body into that chain.

Coloca o meu corpo nessa cadeia.

The first link already half failed: instead of a testis, an . The second link has an enzyme in the middle of the road, , which converts much of the testosterone into estradiol before it gets anywhere. And the third link is my : the muscle's lock opens badly, and what little testosterone arrives gets read by half.

Three links, three losses. The Y is there, and its message never reaches the muscle.

The most extreme version of this is well documented. Women with the complete form of androgen insensitivity have an XY karyotype and testosterone levels that, on paper, are a man's. And even so, that testosterone doesn't translate into what it would do in a body with a working receptor. Peter Sönksen and colleagues, in their analysis of the controversy in sport, describe those levels as extraordinarily high and largely without function. Testosterone without a receptor is a letter with no addressee.

In Part 1 I wrote that I had to relearn how to walk, because the muscle that depended on testosterone went away and my gait no longer worked the same. The airport suitcase is the same phenomenon a year later, in an even more ordinary gesture. My body let go, one by one, of the things it only did because a signal was being read, a signal it no longer reads.

In The Errata No One Read I covered what happens when a sports federation uses a genetic shortcut to decide who competes where, and I won't repeat the legal part here. Here is only the inside version. A shortcut that reads the Y and stops there would hand me an advantage I don't have. It would read the first link in the chain and assume the others worked.

And that answers, once again, the "single case" objection. I'm not saying every XY body is weak, or that every intersex body is like mine. I'm saying something about mechanism, which applies to any body: strength is not a property of the chromosome. It's the end of a chain. When a link fails, what's at the start of the chain stops predicting what comes out at the end.

My Y is real. The strength it was supposed to bring isn't.

O meu Y é real. A força que ele deveria trazer, não.

Chapter 5Capítulo 5

It's Not All RosesNem tudo é flores

Up to here, this text has talked about my body with a certain admiration. The gonad that tied, the uterus that stayed, the that converts. All of that is true. But I need to make a confession before going on, because a text that only shows the pretty side turns into propaganda, and I already said in the prologue what I think of propaganda.

It's not all roses.

Nem tudo é flores.

The previous chapter explained why the Y didn't turn into strength. This one tells what that costs me.

I lost a lot of strength. Lifting weight without wrecking my arms is now a calculation I make before I try. I lost stamina: tiredness comes sooner, in everything, and it comes in the whole body. I lost breath under physical effort. When I push myself, my body takes longer to come back to normal. And there are things I used to do without thinking that I don't do anymore.

Perdi fôlego no esforço físico.

The example that says it best is BGS, Brasil Game Show, the big gaming convention in São Paulo. An event like that is a whole day on your feet, walking, talking to people. Today, after about four hours, my legs are already asking to stop.

And there's a more intimate part. When I get tired, and I get tired of everything faster, my body asks for affection. To be held. Softness. I don't really know why. I just know it asks.

Colo. Fofura. Eu não sei bem por quê. Só sei que pede.

I went looking for an explanation. What I found isn't a closed answer. It's a set of mechanisms that help make sense of it.

Strength, the previous chapter already explained: testosterone's message arrives by half, and what little arrives I block, because it hurts me. Less message, less muscle, less strength for the same weight.

Breath takes a different route, and this one surprised me. Testosterone also tells the body to make more red blood cells, the cells that carry oxygen. It does that in two ways: it stimulates erythropoietin, the hormone the kidney uses to order new blood, and it frees up iron for that production by switching off a protein called hepcidin, which keeps iron locked in storage. And that second route runs through the , the same one that in my body reads by half. Less testosterone action, less hemoglobin, less oxygen reaching the muscle.

The studies that measured this in adult bodies were done with trans women on hormone therapy, and the review by Joanna Harper and colleagues brought twenty-four of them together. I take only the mechanism from them, because my story is a different one. In those studies, hemoglobin drops into the female range in about four months, and strength and muscle mass fall clearly after a year.

The slow recovery comes from the same place, because rebuilding muscle after effort is work testosterone usually helps with. And my legs at BGS put all of that together in a body that's 2.08 m tall (6 ft 10 in): more body to carry, with less muscle and less oxygen to do it. That last part is a possible explanation, not a study.

And being held? Here the science gets more modest, and so do I. There's a type of nerve fiber in the skin, the C-tactile fiber, that responds to slow, gentle touch, the touch of affection, and not to pressure or pain. Stimulating those fibers lowers the body's arousal. Affectionate touch from someone close tends to lower cortisol, the stress hormone, and raise oxytocin. And estrogen acts on the oxytocin system, on how much of it is made and on its receptors. Put the pieces together: a tired body, with more estrogen, may be asking for the cheapest way it knows to switch off the alarm.

E o colo? Estimular essas fibras baixa a excitação do corpo.

It's not a diagnosis. It's a hypothesis that makes sense.

Não é um diagnóstico. É uma hipótese que faz sentido.

And now the part that matters. I didn't choose this price. Nobody asked me whether I'd trade the strength to carry a suitcase for a body that finally went through the puberty it had been waiting for. The body decided first, and it decided without consulting me about the cost.

But I accept it. Without regret.

Mas eu aceito. Sem arrependimento.

And that's why this chapter had to exist. A thesis that says the body decided first is only honest if it shows the bill too. The body decided, and decisions have a price. I pay mine, and I pay it knowing what for.

Eu pago o meu, e pago sabendo o quê.

It's not all roses. The thorns are mine too.

Nem tudo é flores. Os espinhos também são meus.

Chapter 6Capítulo 6

Gonads, Risk, and What the Literature SaysGônada, risco e o que a literatura diz

This is the chapter where I have to be more careful than in any other, because it sets up an accusation I know is coming.

In Part 1 I wrote that, for decades, intersex babies had healthy gonads removed out of an unfounded fear of cancer. I still think that. In January, I'm having mine removed. Among the reasons are how much pain I'm in and the risk of cancer.

So let's do this properly: with numbers.

Então vamos fazer do jeito certo: com número.

The most cited table on this came out alongside the 2006 consensus, the same one that retired the word "hermaphrodite." It sorts intersex conditions by risk of germ cell tumor, the kind of cancer that arises in the cells that would have become eggs or sperm. And it's revealing because it doesn't treat intersex as one thing.

sits in the low-risk group: about 3 percent, an estimate drawn from more than four hundred patients. The complete form of androgen insensitivity is low risk too, around 2 percent. But the partial form, , when the gonad sits outside the scrotum, lands in the high-risk group, with an estimate of 50 percent. And here comes the honesty the table demands: that 50 percent came from very few cases, twenty-four patients. The review by Martine Cools and colleagues, in Rotterdam, which served as the basis for the table, had reached about 15 percent for PAIS. A report by the Australian Senate, Involuntary or coerced sterilisation of intersex people in Australia, pointed out where the difference comes from: with a third study, which was left out of the table, there were eighty patients and 15 percent; without it, twenty-four patients and 50 percent. And the number that circulates as "one in three by age fifty" is something else again: it comes from a 1976 analysis by Manuel and colleagues, based on a handful of historical cases, and it's about the complete form of insensitivity, not the partial one.

Notice what that means. If the report is right, even the table medicine uses to decide surgeries on intersex bodies inflated the fear in a slice that few people checked. That's the kind of thing Part 1 denounced. So I go with the most conservative and best-grounded number: somewhere around 15 percent for PAIS. That's still a lot.

And within ovotesticular DSD itself, tumors show up more often when there's a Y chromosome in the karyotype.

And there's a reason why. Germ cells, the ones that would one day become eggs or sperm, need a typical environment to mature on schedule. In a , or in a body with little androgen action, many of them stay stuck in a fetal state, with stem cell markers like OCT3/4 switched on long after they should be. It's in that state that some of them can turn into the lesion that comes before cancer: germ cell neoplasia in situ, , in testicular tissue, or gonadoblastoma, in tissue that never got to differentiate. That's where the Y comes in: malignant transformation seems to require a specific region of it, called GBY, the gonadoblastoma locus on the Y, where one gene, TSPY, is the most likely candidate. Without that region, the risk plummets. With it, the cell stuck in time has the missing piece.

So where does my body land on that table? Not on one line. On several. I'm ovotesticular, which on its own would be low risk. I have PAIS, which isn't. I have a Y, which counts against me within ovotesticular DSD itself. And the ultrasound of my gonads showed microlithiasis, which means tiny calcifications inside the tissue, and mild atrophy.

E onde o meu corpo cai nessa tabela?

Microlithiasis, on its own, is a finding medicine still argues about. In men without any intersex condition, its link to cancer is uncertain. But a Danish biopsy study found a detail that matters to me: when microlithiasis comes together with atrophy, the risk of GCNIS, the lesion that comes before testicular cancer, is higher. Microlithiasis and atrophy, together. The exact two words on my report.

Exatamente as duas palavras do meu laudo.

None of these numbers is my number. There's no table for an XY person, ovotesticular, with PAIS, microlithiasis and atrophy, diagnosed at 25. What exists is a sum of factors that the literature, each on its own, links to higher risk. When I say my risk is increased, that's what I mean. Not a hunch. A sum.

That leaves the objection, built the way it's going to come.

Falta montar a objeção do jeito que ela vai vir.

"You're romanticizing a malformation. You spend an entire text calling your body a natural variation, a body that decided on its own, a microscope slide. But that same body hurts, needs surgery, and carries cancer risk. That's not variation, it's disease by another name. Calling it natural is activism, not biology."

It's a good objection. It has only one error, and it's a category error.

É uma boa objeção. Ela tem um erro só, e o erro é de categoria.

Natural doesn't mean healthy. Natural means nobody caused it. Cancer is natural. Nearsightedness is natural. The distinction that matters isn't between natural and diseased, it's between needing care and needing to be corrected. A body that needs care gets treatment for whatever threatens its life. A body that needs to be corrected gets treated as a factory defect, and the correction is done to make it look like the model, whether it threatens anything or not.

Part 1 already argued this. The violence I denounced there wasn't treating intersex bodies: it was correcting them without asking anyone, and often out of fear without numbers. What I'm about to do is take care of a measured risk, in a gonad with findings, and the one deciding is an adult who read the numbers.

My variation is natural. So is my risk.

A minha variação é natural. O meu risco também.

Fear without numbers is what Part 1 condemned. Numbers without fear are what I bring.

The Body Decided FirstO corpo decidiu primeiro

Chapter 7Capítulo 7

Before the First DoseAntes da primeira dose

There's a sentence from Part 1 that's going to be used against me, and that's why it's the first thing I need to protect: my body started feminizing on its own, with no outside hormones.

It was true. And it's still true about those months, even though I use hormones today. One doesn't erase the other. So before talking about medication, I want to put on record, calmly, what the body did while nobody was touching it.

The feminization started in November 2025. External hormones only came in June 2026. That's seven months in which the body moved on its own, and it didn't move a little.

The muscle went away. Not the kind of strength someone loses when they stop training, which comes back when they go back: the mass that only existed because a signal was being read, and that vanished when the signal stopped arriving.

The fat changed address. It left where it usually sits in a male body and moved to the hips, the thighs, the pattern medicine calls gynoid. Sex hormones decide where fat goes, and estrogen is what draws that pattern.

The breasts grew. Breast tissue responds to estrogen, and it responds in a way that doesn't reverse when the stimulus stops.

As mamas cresceram.

My genitals changed. They shrank drastically, shifted position after the pelvic tilt, and became more internal, among other things. And orgasm changed along with them: less localized, spread through the whole body, and multiple. When the way pleasure arrives changes, something much deeper than appearance has changed.

A genital mudou.

And the labs kept pace with all of it. Testosterone falling. Estradiol high, and high for months, not a single spike you could blame on lab error. Sustained.

E os exames acompanharam tudo isso. A testosterona caindo. Sustentado.

Notice a detail that usually slips by. Every one of those changes needs two things: estrogen, and a receptor that reads estrogen. My reads by half. My estrogen receptor reads just fine. The body wasn't simply short on testosterone. It was tuned to another station, and hearing it clearly.

I call it delayed puberty, and the word isn't decoration. Puberty, in biology, is this: the moment the gonads wake up and start producing sex hormones in quantity, and the body reorganizes in response. Most people go through it between ten and sixteen. My body went through it at twenty-five. And it went in one direction only.

This matters for a reason that will come up in almost all of the rest of this text: when the outside hormone arrived, it didn't find a male body to transform. It found a body already in the middle of a female puberty, with breasts, with fat in its new place, with muscle going away, and with the high estradiol the body itself had been producing.

The medication didn't open the door. The door was already open, and the body had already walked through it.

The objection that lives here is that, since June, it's become impossible to separate what is body from what is pharmacy. I'll build that objection in full in chapter 9, in the strongest form I can. For now, only the record: before the first dose, the body had already decided.

Chapter 8Capítulo 8

The Hormone That Shuts Me DownO hormônio que me apaga

Part 1 told my diagnosis as if it had been a single appointment. That was a summary. The full order is this.

I went to the hospital in Tokyo because of pelvic pain and left without an answer. Then they called: my hormones were in the female range, and they wanted to see me again. I went back, and the diagnosis came. Notice the irony: what discovered my body wasn't an eye, it was a blood test. And it was on that same trip, after the diagnosis, that the part I'd never told happened.

The Japanese protocol wanted to stop the feminization.

O protocolo japonês quis parar a feminização.

From their point of view, the idea had textbook logic: if the body is feminizing and the karyotype is XY, give testosterone and put the body back on track. They gave me testosterone.

Eles me deram testosterona.

An hour after the dose, I fainted. I spent a whole day in the hospital bed. And it wasn't just that once. They tried more than once, to see whether my body would respond, and the response was always the same family of thing: at the very least, I vomited. When it wasn't that, I'd drift into lowered consciousness, or faint again.

Then they ran a full hormone panel by , which is the most precise method there is for measuring steroid hormones. The difference matters. Most testosterone tests use antibodies, which recognize the molecule by its shape, and similar molecules fool the antibody. At low levels, the error gets as big as the result itself. Mass spectrometry identifies each molecule by its weight, with a precision of less than one atomic mass unit. Not by chance, the Endocrine Society recommends that method for people with low testosterone.

A diferença importa.

And the result left me stunned: the testosterone they had injected hadn't even reached the expected level. had transformed the hormone along the way.

A tinha transformado o hormônio no caminho.

Think about what that means. They gave me the hormone that was supposed to stop feminization, and my body turned much of it into estrogen, which is the hormone of feminization. The drug that was supposed to put on the brakes became fuel.

The explanation from the endocrinologist who saw me was blunt. My body, for some reason, treated testosterone as if it were poison. And with , it couldn't read it properly. In his words, it wasn't important to my body.

Aromatase is an enzyme encoded by a gene called CYP19A1, and its job is converting androgens, like testosterone, into estrogens. That's not a quirk of mine; it's textbook biochemistry, and it's so central that people born without working aromatase have their whole body marked by the lack of estrogen. My quirk is how hard it works. Why my body reacts to testosterone by fainting, I'm still investigating, and I won't invent a mechanism no doctor has given me.

A peculiaridade minha é quanto ela trabalha.

The recommendation I got on the way out was to stay away from any source of testosterone and go back to Brazil. The reason they gave me was that in Japan they didn't treat this.

Fig. 3The Tokyo protocolO protocolo de TóquioHold the button to choose the dose and release to apply (or hold Space). Three doses.Segura o botão pra escolher a dose e solta pra aplicar (ou segura Espaço). São três doses.

The protocol wanted to stop the feminization with testosterone. With each dose, aromatase turns much of it into estrogen, and the feminization meter goes up instead of down; the little that lands fills the malaise meter. On the third dose, the recommendation: stay away from any source of testosterone and go back to Brazil.O protocolo quis parar a feminização com testosterona.

I felt anger, but not at the doctors, who aren't to blame. At the health system. The problem here isn't a person. It's the script. A protocol that looks at the karyotype and decides what the body should be treats testosterone as a neutral hormone, one any XY body takes back. Mine didn't.

And here's the first thing I want you to hold on to, because it comes back in chapter 13. After that episode, I refused any method of stopping my delayed puberty. Nobody talked me into it. My body showed me, and I signed off on it.

Ninguém me convenceu disso. O corpo me mostrou, e eu assinei embaixo.

Medicine tried to put my body in reverse. The body refused before I did.

A medicina tentou engatar a ré. O corpo recusou antes de mim.

Chapter 9Capítulo 9

Then Why HormonesPor que hormônio, então

This is the objection that usually comes first, and it deserves to be built without discounts.

"All of Part 1 rested on one sentence: no external hormones, the body deciding on its own. Since June you've been using estradiol and an antiandrogen. So today it's impossible to separate what is your body from what is the medication. Your so-called spontaneous puberty becomes a convenient legend, told to give the process an air of nature it no longer has. If the body was deciding on its own, why did it need a pharmacy?"

It's a good question. The answer has three parts.

É uma boa pergunta. A resposta tem três partes.

The first is already in chapter 7: before the first dose, the body had already done the bulk of the work. Breasts, fat in its new place, muscle going away, high estradiol sustained for months. The pharmacy came in after the body, not in its place.

The second is in chapter 8: the pharmacy already tried to take charge of my body, and lost. When a protocol tried to push my body in the opposite direction, with testosterone, the body answered by fainting, and the dose didn't even reach the expected level. If an outside hormone could decide my sex, it would have decided it in Tokyo. It didn't.

The third is the question the objection forgets to ask: why did the doctors swap the body's own hormone for an outside one?

Predictability. A body that makes estradiol on its own, from tissue that shouldn't even be doing that, makes it its own way: with variation, with swings, with no instructions on the label. An outside hormone has a known dose, schedule and level. The doctors preferred to swap an unpredictable engine for one they can measure. Not because the body wasn't responding, but because it responded without telling anyone how much.

Previsibilidade.

Fig. 4Noise and a steady sourceRuído e fonte estávelKeep the trace inside the band: drag the droplet up and down, or hold ↑ or ↓. Two rounds. Duas rodadas.

You try to keep a level inside a band, twice. In the first round the source is the tissue, which makes it its own way: it responds, but swings without warning, and your hand always arrives late. In the second the source is gel on the skin, with the dose split across the day: the variation is small and you can see what is coming, because dose, schedule and level are known. At the end the two shares of time inside the band sit side by side. The doctors did not swap the engine because the body wasn't responding, but because it responded without telling anyone how much. The outside hormone is a stabilizer and an amplifier, not the engine.

What I use today is estradiol gel, on the skin, and an antiandrogen, cyproterone. And I split the gel dose across the day, to keep the level as continuous as possible. Even how I use the medication is about that: taking out the ups and downs. Before cyproterone I used an injectable blocker, leuprorelin, which shuts off the command the brain sends to the gonads. The switch was my doctors' call, for the flexibility of adjusting the dose close to surgery.

The gel and the split dose have pharmacology behind them. Estradiol in pill form passes through the liver first, before reaching the rest of the body, and on that pass the liver ramps up production of clotting factors. Through the skin, the hormone goes straight into circulation, without that first pass, and the effect on clotting is much smaller. And gel is absorbed gradually, with a peak a few hours after application and a drop afterward. Splitting the dose into more than one application flattens the peak and fills in the drop.

And here a biology reader will call me out, fairly: if doesn't read testosterone, and turns testosterone into estrogen, why block testosterone?

Because PAIS isn't zero. It's partial. Testosterone works on me by half. The part the receptor doesn't read, aromatase turns into estrogen. But the part it does read hurts me, like poison: my own testosterone gave me symptoms similar to the ones in Japan. That's the part cyproterone blocks. It does two things at once: it occupies the in testosterone's place, and, because it acts somewhat like progesterone, it lowers the signal telling the gonads to make testosterone.

The doctor in Japan said testosterone wasn't important to my body. I say the part that arrives hurts me. Both things are true at once.

As duas coisas convivem.

So what is the outside hormone in my body? I don't think it's the engine. The engine started on its own in November 2025. For me, the external hormone is a stabilizer and an amplifier: it holds the level the body had already chosen, without the swings of tissue that works without instructions, and it reinforces the direction the body had already taken. And there's another reason for it to be there, which has to do with what happens if it leaves, and which I'm saving for chapter 15.

The pharmacy didn't decide anything. It arrived afterward, held the volume steady, and turned up a song that was already playing.

Chapter 10Capítulo 10

Open Growth Plates at 26Placas abertas aos 26

Now a chapter to catch your breath, because this body has its ironies too.

I'm 2.08 m tall, 6 ft 10 in. And at 26, my are still open.

Eu tenho 2,08 m. E, aos 26 anos, as minhas continuam abertas.

A growth plate is the band of cartilage near the end of the long bones, where bone grows in length. In adolescence it closes, the bone fuses, and growth ends. The interesting question is: who tells it to close?

A pergunta interessante é: quem manda ela fechar?

Until 1994, medicine's answer was intuitive and wrong. In male bodies, the thinking went, testosterone was what closed the plates. Then came a case that turned the book inside out, described by Eric Smith and colleagues in the New England Journal of Medicine. A 28-year-old man, 2.04 m tall (6 ft 8 in), with open plates, still growing in adulthood, with a completely normal male puberty. He had plenty of testosterone. What he didn't have was a working estrogen receptor: a mutation had switched off the alpha receptor. And even with estrogen ten times higher in his blood during the study, his body didn't respond.

New England Journal of Medicine.

A year later came the cases of deficiency described by Akira Morishima and colleagues: men who couldn't make estrogen, and who were very tall, with open plates and fragile bones. The conclusion became consensus: what closes the growth plate is estrogen, in men too. Men's testosterone closes the plates largely because aromatase turns part of it into estrogen.

I won't say my case is the same as those men's, because it isn't. He kept growing; I stopped at 21. But the mechanism is the same one that makes my open plates at 26 make sense: during the years when a typical body would have had its wave of sex hormones, mine didn't have enough estrogen to tell the plates to close. The puberty that would have closed them arrived at 25. Late, like everything else.

And the open plates don't only explain my height. They explain why my skeleton is still changing shape, and that's not theory. These are changes in my body.

The curve at the base of my spine, the lumbar lordosis, deepened into the female pattern. My hips widened. And my pelvis tilted forward, the pelvic tilt, which changed how my spine meets my hips and even changed the position of my genitals, as I described in chapter 7. Those are three changes a typical adult skeleton, with closed plates, would no longer make this freely. Mine did, because it's still open.

A curva da base da coluna, a lordose lombar, aumentou e ficou no padrão feminino. O quadril alargou.

The mechanism exists. A study by Marcia Ponce de León's team at the University of Zurich, which followed the development of the human pelvis from birth to old age, showed that until puberty, boys' and girls' pelvises follow almost the same path. At puberty, the female pelvis changes course and widens until it reaches its maximum width between 25 and 30, and the authors link that change to estrogen. The spine has a sex pattern too: the female lower back tends to have a deeper curve, and a forward-tilted pelvis goes along with that curve. My body took that route about ten years late, in a skeleton that stayed open precisely because the estrogen hadn't arrived yet.

O mecanismo existe.

And there's something I like about all this: I like my height. If I had started estrogen at 18, my plates would have closed sooner, and I'd have ended up shorter. I wouldn't trade it.

I get comments about it, and most of them are disbelief or astonishment when people see me, as if a 6 ft 10 in woman were a contradiction. It isn't. It's a body that got the signal to stop growing late, because the signal that closes the plate is the same one it took so long to produce.

My skeleton waited for puberty too. Except that, unlike the rest of me, it made good use of the wait.

Chapter 11Capítulo 11

The Dysphoria That Gave WarningA disforia que avisava

In Part 1, I wrote that gender dysphoria still got in the way more than the good part helped. That was true at the time. Today, about my body, it isn't.

Before I explain, I want to tell you something I noticed over these months.

The dysphoria didn't hit my whole body at once. It showed up in one region. A specific part of my body would start to bother me, to feel wrong, to ask for something I couldn't name. And a few weeks later, the body changed right there. And the dysphoria in that region went away.

E a disforia daquela região sumia.

I say "a few weeks" because that's how I lived it, not because I timed it. At first I said two weeks, and that's an approximation, not a measurement. What I know for certain is the order: first the discomfort, then the change, then the quiet.

Fig. 5The warningO avisoMark the lit region before it changes. Arrow keys and Enter work too.Marca a região acesa antes que ela mude. Setas e Enter também servem.

In an abstract field of regions, one lights up: that is the warning. Some time later, with no fixed delay, it changes shape and texture right there, and the light goes out. First the discomfort, then the change, then the quiet. Whoever learns to read the light predicts each change before it happens, and the field ends fully changed and quiet. It wasn't a desire against the body. It was the body giving warning.

I don't have a lab explanation for this, and I won't pretend I do. I have a clue, and only a clue. Change in the body starts on the inside before it shows on the outside. A breast bud, for example, usually becomes sensitive and sore before it grows. And the brain is constantly receiving signals from inside its own body, what science calls interoception, many of them before anything becomes visible in the mirror. Maybe the discomfort was that: the body feeling the construction work before it showed.

I'm not saying dysphoria is tissue pain. I'm saying the place where it showed up was the place where the tissue had already started working. It was like being halfway there without having arrived, and that's what caused my dysphoria. The clue doesn't settle the question. But it makes the warning less mysterious.

And the pattern on its own changes the meaning of a word everyone thinks they understand. Dysphoria is usually described as an identity fighting a body: the person feels they're one thing, the body is another, and dysphoria is the friction between them. In my case, what I lived was different. The dysphoria didn't fight the body. It arrived ahead of it, in the exact place where it was about to change.

Ela chegava antes dele, no lugar exato onde ele ia mudar.

It wasn't a desire against the body. It was the body giving warning.

Não era um desejo contra o corpo. Era o corpo avisando.

That has a consequence that will weigh in chapter 16, when I answer the accusation that I'm "trans intersex." In Part 1, I wrote that I was already living with dysphoria before the diagnosis, and that it worked as a shield when the diagnosis came. Anyone who wants to use that against me will say identity came before the body. The pattern in this chapter says something else: the dysphoria always came before the change, and the change always came.

Today, the dysphoria about my body is over. What's left is social: it's how other people read me, not how I feel inside myself. That gets a whole block further on, the one about being read.

The dysphoria followed the body, not the other way around. When the body finished arriving, it left.

I Decide AlongsideEu decido junto

Chapter 12Capítulo 12

The Paper Switched SidesO papel mudou de lado

In Part 1 there's a sentence that closes one of its most personal passages: "My body was always female. What decided otherwise was a piece of paper."

The paper switched sides. My ID card, the Brazilian RG, now says F.

O papel mudou de lado. O meu RG agora diz F.

I want to stay with that fact for a moment, because it looks bureaucratic and it isn't. For twenty-five years, a document stated something about my body based on the only layer you could see on the day I was born. Chapter 2 showed what was left out of that reading: a uterus, ovarian tissue, a gonad that wasn't what it seemed. The paper didn't lie in bad faith. It just recorded a guess as if it were a fact. I never agreed with that document. I just didn't have the courage to do anything about it before I found out I was intersex.

Now it agrees with the body.

Agora ele concorda com o corpo.

That may seem small to anyone who never had to think about their own ID. A document gets read before the person does. At the bank counter, at check-in, at the hospital front desk, someone looks at the paper before they look at whoever is standing in front of them, and it's the paper that tells them how that person will be treated for the next few minutes. For every institution, the first reading of who I am has always been the document's.

That's why an F in place of an M isn't cosmetic. It's the first reading starting to agree with the second. Whoever looks at the paper and then looks at me no longer finds two answers fighting each other.

What that F didn't do counts too. It didn't change a single tissue in my body. What it changed was the outside of the conversation: how the world reads me before it sees me.

Notice the order as well. The body didn't change to match the new document. The document finally caught up with a body that had already gone ahead. That looks like a detail of sequence, and it's the whole thesis of this text in miniature: the body decides, and the rest arrives later. The paper was the last to know.

And this is where the text turns. Up to this point, the body did the deciding: the gonad that tied, the uterus that stayed, the that converted, the receptor that read by half, the puberty that arrived at twenty-five. From here on, something comes in that didn't exist in Part 1. My signature comes in.

Entra a minha assinatura.

The paper caught up with the body. Now I'm the one catching up with both.

Chapter 13Capítulo 13

Not the Scalpel, the SignatureNão é o bisturi, é a assinatura

My first signature wasn't a yes. It was a no.

A primeira assinatura não foi um sim. Foi um não.

It was in Tokyo, in chapter 8. A protocol tried to stop the feminization with testosterone, my body answered by fainting, and after that I refused any method of stopping my delayed puberty. Nobody had asked me to sign anything yet. I signed anyway, with a refusal.

Ninguém tinha me pedido pra assinar nada ainda. Eu assinei mesmo assim, com uma recusa.

The yes came later, and it comes in January.

In January, I'm having a : the removal of my gonads. In the same operation, the team will create a . The team wants both in a single operation, and their own words sum up why: "we're not opening you up twice."

Before I explain why, I need to do something few people do with their own writing. I need to build the case against myself in my own words.

"In Part 1, you wrote that intersex babies underwent normalizing surgeries without consent, and you listed among them the removal of healthy gonads out of unfounded fear of cancer. You cited the UN Special Rapporteur on Torture, who in 2013 classified those surgeries on intersex children as a form of torture and cruel treatment. Now you're having your gonads removed, and among your reasons is precisely the risk of cancer. And your scan showed microlithiasis, which is exactly the kind of finding that has justified many of the surgeries you condemned. That has a name: hypocrisy."

It's the strongest objection there is against this text. It deserves an answer that doesn't dodge any part of it.

The answer starts with a word the accusation skips: without. Part 1 never condemned surgery. It condemned surgery without the person. Surgery done on a baby who can't say yes, or no, or "wait." The violence wasn't in the scalpel. It was in whoever held the pen.

A parte 1 nunca condenou a cirurgia.

A baby doesn't sign. I do.

Um bebê não assina. Eu assino.

Bioethics has a name for what separates the two. For a signature to count, three conditions have to exist at the same time: capacity, which is being able to understand and decide; information, which is knowing what will be done, with what risks and what alternatives; and voluntariness, which is deciding without coercion. A baby lacks the first, so none of the others ever gets to matter. I have all three: I'm an adult, I've read the numbers in this text, and nobody pushed me onto the operating table. It was the opposite. The only protocol that tried to push me, in Tokyo, pushed the other way, and I refused.

The second part of the answer is in the adjective the accusation also skips: healthy. Part 1 was about healthy gonads. Mine aren't. They hurt: severe pain, the kind tramadol gets prescribed for, in the genital and pelvic region. They have findings on ultrasound: microlithiasis and mild atrophy. And they carry an increased cancer risk, which chapter 6 showed with numbers, not with fear.

The third part is that: the numbers. Part 1 condemned unfounded fear, and I still condemn it. My risk enters this text with the table, with the margin of error, with the correction of a number medicine itself inflated. What I denounced was fear that skipped the math. Here the math comes first.

A terceira parte é essa: o número. A parte 1 condenou o medo infundado, e eu continuo condenando. Aqui a conta vem primeiro.

And finally, the sentence that sums up my decision better than any technical argument: I'm not going to leave something in my body that gets in the way of my life.

Now, the part the accusation doesn't even get to, and that shows up right after it. If the gonadectomy is clinical, what is the neovagina?

The main stage of the surgery is the gonadectomy. That's the clinical part. The neovagina is alignment with my identity and function, and it also resolves a genital pain. And it doesn't create something out of nothing. It completes what the body already started: a genital feminization so strong it changed everything, with the pelvic tilt, with the clitorization. My body moved in that direction on its own, for months, before any medication. The surgery doesn't open a new road. It reaches the end of a road the body opened.

A etapa principal da cirurgia é a gonadectomia. Ela é a parte clínica.

And what I want from it, I can say in one sentence: I want to be whole and to feel my body. I won't accept "half."

An abstract procedure waits, stopped, in front of a gate. Time passes and it doesn't move; scrolling doesn't move it either. Only a signature opens the gate, and it counts when three conditions exist at the same time: capacity, information and voluntariness. When you sign, the procedure goes all the way through, and your signature stays as what made it move. It's not the scalpel that separates violence from care. It's the hand that signs.

Two more objections live here, and both deserve the same honesty.

Aqui moram mais duas objeções, e as duas merecem a mesma honestidade.

The first: "And what if you regret it? It's an irreversible surgery, at twenty-six, in a body that's still changing and has open . Waiting would be the bare minimum of prudence."

Waiting for what, exactly? The changes in my body don't regress. There's no male body stored somewhere for me to go back to. If the hormones stop and the body can't sustain itself, and chapter 15 explains why that can happen, what follows isn't detransition. It's . The dysphoria about my body is over. And the person deciding isn't a teenager alone on an impulse: it's an adult, with a team, with medical reports, with test results, and with time.

The second objection is sharper, and it uses the previous answer against me: "If your dysphoria about your body is over, then the neovagina treats nothing. It's elective, almost cosmetic. Either the dysphoria is over and the surgery is unnecessary, or it isn't over and your argument against regret collapses."

It's a good dilemma. It only works if the surgery were a treatment for dysphoria. In my case, it isn't. I don't have dysphoria about my body anymore. What I have is pain. The pain comes from the gonads, and that's where the indication comes from. The absence of dysphoria supports that there won't be regret. The pain supports the gonadectomy. And the neovagina, which happens in the same operation, stands on the rest: on function, on the genital pain it also resolves, on the body it completes.

A dor sustenta a gonadectomia.

And if someone insists that, even so, part of it is a choice, I agree. It's a choice. And that's why it's the cleanest proof of this text's thesis. An informed adult doesn't need dysphoria for her signature to count.

What happens in January, and what comes after, is for chapter 24. Here, the why.

It's not the scalpel that separates violence from care. It's the hand that signs.

Intersex Is Not TransIntersexo não é trans

Chapter 14Capítulo 14

The Difference, AgainA diferença, de novo

Part 1 had a whole section on this, and I'm going to do it again, because, as I said there, it's one of the most common confusions on the subject, and because my case has made the distinction sharper than it was.

Intersex is about the body. It's a variation in any of the five layers from chapter 1: chromosome, hormone, receptor, gonad or anatomy. It's a biological description.

Trans is about identity. It's the situation of a person whose gender identity doesn't match the sex they were assigned at birth.

They're different questions. One asks how the body was made. The other asks how a person recognizes themselves in relation to what they were told they were. A person can be intersex and not trans. A person can be trans and not intersex. And there are people who are both at once. But one doesn't create the other. And Part 1 already said why mixing them up harms both groups: it erases what's specific to the intersex struggle, which is autonomy over our own bodies, and it feeds people who want to use intersex bodies as ammunition against trans people, or in their favor, without asking anyone anything.

And there's something I think now. Labels like trans, cis and intersex make sense in a medical context. There, they're good for something: they decide tests, risk, protocol, surgery. In daily life, they only segregate. On the street, I don't want to be a category. I just want to live as a woman, no suffix.

Which takes me straight to the objection that seems to take all of this apart at once.

"If labels only segregate in daily life, why do you fight so hard not to be called trans? You say yourself these labels shouldn't matter outside the doctor's office, and yet you write a whole block, in a public text, to refuse one of them. Either the label matters or it doesn't. Pick one."

Escolhe."

I pick both, and they don't contradict each other.

First, because the trans label is uncertain in my case. Trans presupposes a mismatch between identity and assigned sex. Chapter 16 will show why, in my body, that mismatch was never with the body: it was with the paper.

Primeiro, porque o rótulo trans é incerto no meu caso.

Second, because of the stigma "trans" carries. And here I want to be precise, because it's easy to misread: it's not that trans people should be ashamed of anything. It's that if I can use intersex, or nothing at all, I'd rather. And, fortunately or unfortunately, society understands intersex, or hermaphrodite, better than it understands trans.

This text isn't fighting for a label. It's refusing one that doesn't describe my case. Wanting labels to matter less doesn't oblige anyone to accept the wrong label while they still matter.

Chapter 15Capítulo 15

So It's Transition?Então é transição?

This objection doesn't come from someone who didn't read. It comes from someone who read everything so far and put the pieces together in a way that seems to fit.

"Look at the checklist. Feminizing hormones. A blocker. Genital surgery on the calendar. Care at NuMA, a clinic focused on LGBTQIA+ health and on hormone therapy for trans people. That is, item by item, the protocol of a transition. Calling it intersex is taking the one door that escapes the stigma of being trans. The body may well have started on its own, but what you're doing now has a name, and the name is transition."

"Olha o checklist. Hormonização feminizante. Bloqueador. Cirurgia genital marcada.

My answer fits in one sentence.

A minha resposta cabe numa frase.

It isn't transition, because there's no binary to go back to. Every change is irreversible. And if, by any chance, the hormones stop, I go into , not detransition.

That sentence has three parts, and each one takes apart a piece of the checklist.

"There's no binary to go back to." Transition presupposes a starting point: a body of one sex moving toward the other. My starting point was an , a uterus nobody saw, a receptor that reads by half, and a chromosome that didn't win the tug of war. There's no original male body I'd be leaving. There's a body that was never just one thing.

"Every change is irreversible." What the body did on its own between November 2025 and June 2026 doesn't go back. It's not a phase that undoes itself when the medication stops.

"If the hormones stop, I go into hypogonadism." And here I need to explain why that holds today, not just after surgery. My labs show high . FSH and LH are the hormones the brain uses to tell the gonads to work. When they're high, it means the brain is asking and the gonads aren't delivering enough. It's the classic sign of : gonads that formed atypically, back in the embryo, and that over time can't keep up with what the brain asks for.

Think of a thermostat. The brain measures how much sex hormone is circulating, the way a thermostat measures the temperature of a room. If the room is cold, it tells the heater to work harder. FSH and LH are that command. When they're high, the room isn't warming up enough, and the thermostat turns up the demand. The problem isn't the thermostat. It's the heater.

If I take away the outside hormone and the body can't sustain itself, what follows is a body without enough sex hormone, with everything that brings. Not a body that goes back to being a man.

That also answers a question an endocrinology reader will ask, and it's worth building in full: "If your ovarian tissue makes enough estrogen to feminize you, your FSH and LH should be low. The two don't add up. Either the ovary isn't producing, or the test is wrong, and the spontaneous puberty story collapses with it."

They do add up. It's the thermostat again: the heater works, it just can't heat the room. The tissue produces, but less than the brain asks for, and the brain raises the demand to compensate. It's not a contradiction. It's a body that went through puberty with a gonad that could no longer keep up on its own. And that's why the outside hormone, in my body, is a stabilizer and an amplifier, not the engine.

Fecham, sim.

About NuMA: I'm treated there, and I don't hide it. NuMA, Núcleo de Medicina Afetiva, is a São Paulo clinic focused on LGBTQIA+ health, with an emphasis on hormone therapy for trans people. And the reason I'm there is simple: my endocrinologist, Henrique Cecotti, one of NuMA's founders, knows intersex well. But where I'm treated doesn't define what my body is. A gender identity service is, today, the place with a team that knows how to handle hormones, medical reports and surgery in a body like mine. That says a lot about what the health system lacks for intersex people, and almost nothing about me.

The checklist describes the procedures. It doesn't describe the direction. The direction, the body chose before any item on the list.

Chapter 16Capítulo 16

Registered as a BoyRegistrada como menino

This is the best-built objection of all, and I'll assemble it carefully, because it deserves that.

"Trans, by definition, means having a gender identity different from the sex assigned at birth. You were assigned a boy. You lived as a man. Today you live as a woman. The definition fits word for word. Part 1 itself admits there are people who are intersex and trans at the same time. And there's more: in Part 1 you confessed you already had dysphoria before the diagnosis, and you wrote, in so many words, 'I already wanted to transition one way or another.' Identity came before the body. That's precisely the definition of trans. So you'd be, at the very least, intersex and trans. Refusing the label is a preference, not an argument."

A identidade veio antes do corpo. Isso é exatamente a definição de trans. Então tu serias, no mínimo, intersexo e trans. Recusar o rótulo é preferência, não argumento."

Read that objection again, because it uses a sentence of mine. It deserves an answer with the same precision.

Let's start with the definition, which the objection uses as if it were a neutral ruler. "Sex assigned at birth." Assigned based on what? Chapter 2 answered: on external anatomy, one layer out of five, while the other four went unread. My birth was registered with incomplete data.

That changes what the definition measures. When the objection says my identity doesn't match my assigned sex, it's right. But the assigned sex wasn't my body. It was a partial reading of it, turned into a document. The mismatch was never between my identity and my body. It was between my identity and the paper.

What decided otherwise was a piece of paper. I wrote that in Part 1, and this is where the sentence shows what it's for.

And this isn't a thesis that's only mine. Brazilian law itself admitted, in 2021, that the reading at birth may not be enough. Since September of that year, a rule from Brazil's National Council of Justice, now incorporated into the national code of rules for civil registry offices, allows a baby to be registered with sex "unknown" when the live birth declaration comes filled in that way. The assignment can be made later, at any time, at any registry office, without a court case, without surgery, without hormones and without a medical report. Before that, the registry office couldn't issue a birth certificate without a defined sex, and families were pushed into court, or into early surgery to "match" the sex chosen on paper. The rule exists because the system recognized that looking at the outside, on day one, sometimes doesn't tell the truth about the body.

Correcting a record made with incomplete data isn't changing sex, just as correcting a wrong date of birth isn't changing age.

Fig. 7Five seconds and two boxesCinco segundos e duas caixasExamine the shape with the light for five seconds, then check a box. Arrow keys move the light; Enter checks. As setas movem a luz; Enter marca.

An abstract shape of five layers, in the dark. A flashlight lights it for five seconds, and only the outer layer gets read; the inner ones show, but stay unread. In round 1, before 2021, the record has two boxes, M and F, and moves on only once one is checked. In round 2, after 2021, a third appears: unknown. After both rounds, the five layers are revealed, from the outside in: anatomy, gonad, receptor, hormone and chromosome, and your two checks sit side by side.

That leaves the sentence from Part 1. Yes, I wrote that I already wanted to transition one way or another. And the objection reads that as proof that identity came before the body.

Chapter 11 has the answer, and it wasn't built for this chapter: it's what I lived. The dysphoria never arrived as a desire against the body. It arrived in one region, and a few weeks later the body changed in that same spot. The dysphoria I had before the diagnosis was the same phenomenon, before I knew its name. It was the body giving warning before it changed.

And there's a second part, which is simpler. Wanting doesn't define anything. What defines is what happened, and what happened is that the body acted before any decision of mine. Puberty started on its own in November 2025. No desire of mine produced estradiol, or made a uterus appear, or converted testosterone into estrogen. If my body had stayed where the paper said, this would be a different text. It's this one because the body went first.

As for being intersex and trans at the same time: Part 1 said it, and I'll say it again, that it exists. But it isn't automatic, and it isn't my case. A combination existing doesn't oblige anyone to wear it.

The definition of trans measures the distance between an identity and a record. In my case, what was wrong was the record.

Chapter 17Capítulo 17

The FaceO rosto

In chapter 11 I said the dysphoria about my body is over. It's true. What's left is of another nature: it's not how I feel inside myself, it's how other people read me. Social dysphoria.

Disforia social.

And the place where other people's reading happens first is the face.

E o lugar onde a leitura dos outros acontece primeiro é o rosto.

Sometimes I still hear "moçoça" or "moçaço": Portuguese mash-ups of moça and moço, girl and guy, the word someone invents when the eye doesn't know which box to put what it's seeing in. For a while, my face was too neutral for a hurried glance. Not masculine. Neutral.

Neutro.

Notice the difference, because it matters. The bone structure of my face isn't masculine, and I don't have an Adam's apple. The skeleton was already on the right side. What was missing was what sits on top of it.

That's where the hormone did something I didn't expect it to do so well. Estrogen redistributes fat across the whole body, and the face is no exception. Soft tissue shifts, the cheeks gain volume where they usually do in a female face, the features soften. I thought I'd need heavy facial feminization surgery, the kind that works on bone. With the change in soft tissue, the surgery became fine-tuning.

Hair is another story. Facial hair that has already formed doesn't go away with estrogen: the hormone slows it down, but the follicle that androgen activated is still there. That's what laser is for, and I've done seven sessions. And there are several sessions for a physical reason. The laser targets the hair's pigment and heats the follicle until it stops producing, but it only works on hair in the growth phase, while it's still attached to the root. Each follicle on the face is at a different phase of its own cycle, so each session only catches part of them. You have to come back when the next batch enters its growth phase.

There's still one thing I want to fix: my nose. And I will. If the doctor points to any other area besides the nose, I'll go with what he recommends.

All told, my face passes very well today. What lies ahead isn't correction. It's finishing.

The dysphoria that's left is social, and my answer to it is also a signature. I choose what to do with my face the same way I choose what to do with the rest: knowing what the body already did on its own, and deciding what's left.

Chapter 18Capítulo 18

The VoiceA voz

If my face already passes, why touch my voice?

It's the right question, and the answer isn't the one everyone expects.

I've decided to have a , after the . And I decided knowing the price: the surgery affects singing. For someone who sings, that weighs.

Eu decidi fazer uma , depois da . E decidi sabendo do preço: a cirurgia tem impacto no canto. Pra quem canta, isso pesa.

The technique is simple to explain. The vocal folds vibrate like strings, and the pitch of the voice depends, among other things, on the length of the part that vibrates. In a Wendler glottoplasty, the surgeon joins the front part of the two folds, which heal together, and the vibrating stretch gets shorter. Shorter string, higher note. Studies on larynges, on models and on patients show the voice's frequency rising consistently. And they show the other side too: vocal range tends to shrink, and sometimes the voice gets rougher. It's not a surgery without cost. That's why anyone who decides to have it needs to know what they're trading for what.

What I'm trading is this.

O que eu estou trocando é isto.

Today I already pass. My voice doesn't get in the way of how my face passes, which is already very good. The issue is people with very narrow references: people for whom a woman has to sound one very specific way, and anything outside that band becomes a question. And I don't want to have to explain my biology every time. At BGS, I had to.

And here's a fact that changes the conversation. My fundamental frequency, the average pitch I speak at, sits around 180 Hz. For a ruler: men's voices usually fall between about 100 and 140 Hz, and women's between about 180 and 220. In between, roughly 145 to 175, is the band the ear doesn't know how to classify. And research on perception, like King and colleagues' study, shows that from an average around 180 Hz, most listeners already hear a female voice. My 180 sits right at that line, on the right side of it.

My voice barely deepened at puberty, if it did at all. So what gives me away, when anything does, isn't pitch. It's prosody, the melodic shape of speech, which rises and falls differently in men and women. And it's a few mannerisms with drive, the distortion technique in singing, that I used for too long and that I'm unlearning in speech therapy.

Speech therapy handles what I control. The glottoplasty changes the floor of the voice: by taking the lowest notes out of range, it keeps the voice from dropping down low in the moments nobody controls, like laughing, coughing or shouting. When you speak right at the line, any dip crosses it. The two don't compete. One trains the habit. The other changes the instrument.

A outra muda o instrumento.

So I'd rather have every surgery possible, besides the neovagina, which is about function and the end of pain. The Wendler glottoplasty and facial feminization take care of the problem that's left.

Fig. 8Narrow referenceReferência estreitaDrag the edges of each listener's window (or use ← →) and let all seven pass.

The same voice, around 180 Hz, at the edge of the women's band, passes seven listeners. Each one hears it through their own reference window. Wide windows reach 180 and hear a female voice; narrow ones don't, the voice becomes a question, and every misreading adds an explanation given. You widen the windows and the counter stops. The wave never changed: what misreads is the narrow window, not the voice. The majority already reads it right; what's left is not having to explain yourself to the minority that reads it wrong.

Notice what the voice reveals about passing as a whole. If the goal were to pass, I wouldn't need the surgery, because I already pass. The goal is something else: leaving no room for misreading. Not having to introduce myself with a paragraph of biology before I can say my name.

That may look like a small difference. It isn't. Passing is being read correctly by the majority. What I want is not to have to explain myself to the minority that reads me wrong.

And singing? Singing pays part of that bill, and I accept the cost. It's the most expensive signature in this text. That's exactly why it's the most honest: nobody trades a piece of their own singing for vanity.

E o canto?

Chapter 19Capítulo 19

Passing Is Not a SinPassabilidade não é pecado

This objection comes from the side many people would expect to defend me, and that's why it has to be built carefully.

"Laser, facial feminization, voice surgery: all of it is capitulation. It's bowing to a standard of femininity that oppresses precisely the women who can't reach it. Every woman who chases passing reinforces the ruler that excludes the others. Passing isn't freedom. It's obedience."

There's a true part in there, and I won't pretend there isn't: yes, there's a narrow standard of femininity, and it hurts anyone who doesn't fit. I said so in the previous chapter, when I talked about people with narrow references.

The error is in the leap the objection makes next. It takes a real problem, the narrow ruler, and shifts the blame onto whoever decides to adapt to it.

Nobody forces me to do any of this. I chose. And denying a woman the right to choose what to do with her own face and her own voice, in the name of activism against standards, is one more way of deciding over her body. It swaps the hand holding the pen and calls it liberation.

Ninguém me obriga a fazer nada disso. Eu escolhi.

I defend something that gets me hate, and I'll keep defending it: recognition is social, and it runs through the body. Identity alone isn't enough for a woman to be read as a woman. Other people's reading happens in the face, the voice, the manner, before any conversation. Saying that isn't agreeing with the ruler. It's describing how the ruler works, so I can decide what to do about it.

And the voice, once again, dismantles the objection's premise. It assumes I want to pass. I already pass. What I want is not to spend my life explaining my biology to people with a narrow window. That isn't obedience. It's an economy of existence.

In Proof of Dissent I discussed passing from the angle of discourse, and I won't repeat here what's there. Here is only the inside version, the one belonging to someone heading into the operating room knowing why.

Passing is neither a sin nor a duty. It's a choice, and a choice is only mine if nobody, on any side, gets to make it for me.

Chapter 20Capítulo 20

Eunuchs From the WombEunucos desde o ventre

I'm Reformed. I believe in predestination, in God's sovereignty over every detail of creation, and in Scripture as the rule of faith.

Eu sou reformada.

The question my tradition asks of a body like mine usually comes like this, in the voice of a conservative Reformed brother, and I want to build it with the respect it deserves:

"If God is sovereign, and He decreed your body, then intervening in that body is refusing the decree. Hormones, surgery, scalpel: all of it is the creature telling the Creator He got it wrong. The law of Moses itself, in Deuteronomy 23, excluded the mutilated man from the congregation. Whoever tampers with the body they received is rebelling against the one who gave it."

Before answering, I want to read calmly what Jesus said about bodies that don't fit the scheme.

In Matthew 19, the disciples have just heard the teaching on marriage, and Jesus begins like this: "For there are some eunuchs, which were so born from their mother's womb." Then he speaks of those made eunuchs by the hands of other men, and of those who chose that path for the sake of the kingdom of heaven. And he closes with a challenge: whoever can receive it, let them receive it.

Notice where that sentence sits. Eight verses earlier, in the same chapter, Jesus answers the Pharisees by recalling Genesis: in the beginning, the Creator made them male and female. That's the verse most often used to say only two body designs exist. And it's the same Jesus, in the same conversation, who right afterward names those born that way from their mother's womb. The rule and those born outside it are in the same chapter, in the same mouth. Whoever quotes verse 4 against me needs to explain why they skip verse 12.

Read the first category again. Slowly.

Lê de novo a primeira categoria. Devagar.

Those who were born that way.

Os que nasceram assim.

Jesus separates three kinds of people outside the reproductive scheme: those made that way by human hands, those who chose that path for the Kingdom, and those born that way. The third isn't an accident, or a punishment, or a rebellion. Nobody did that to them, and they didn't do it to themselves. They came that way from the womb. Rabbinic Judaism, a few generations later, gave this names. The Mishnah distinguishes the (סריס אדם), the eunuch made by human hands, from the saris chamah (סריס חמה), the eunuch "of the sun," the one born that way. And it goes further: it discusses the androginos (אנדרוגינוס), the body that has both, and the tumtum (טומטום), a word that means "hidden," the body whose sex doesn't let itself be seen. One of the sages went as far as saying the androginos is a creature of its own, and that the sages could not decide whether it was a man or a woman.

Elas vieram assim do ventre.

Jesus names that body without condemnation. He doesn't call it an error to be corrected. He puts it in the same sentence as the kingdom of heaven.

And He wasn't making this up from nothing. He was speaking to people who knew Isaiah. In Isaiah 56, in one of the most beautiful passages of the Old Testament, the prophet addresses the eunuchs, whom the law kept at the margins of the congregation. And the first thing God does is forbid the eunuch from saying of himself: "Behold, I am a dry tree." Then comes the promise: to the eunuchs who keep the covenant, God gives a place inside His own house, and a name worth more than any descendants, a name that won't be erased.

In my essay on The Matrix I defended the affirming reading of that passage, and I stand by it. Here it becomes almost literal. A body the law left outside the door receives, from the mouth of God Himself, exactly what the law had denied.

No ensaio sobre Matrix eu defendi a leitura afirmativa dessa passagem, e mantenho. Aqui ela fica quase literal.

And one of the first baptisms the book of Acts records outside Israel is that of an Ethiopian eunuch, who had been reading Isaiah on the road.

So when someone tells me a body like mine is rebellion against the Creator, I answer with Scripture before I answer with biology. Scripture names bodies like mine. And it names them to welcome them.

E nomeia pra acolher.

The Bible doesn't ignore those born outside the scheme. It gives them a name, a house and a promise.

A Bíblia não ignora quem nasceu fora do esquema. Ela dá nome, casa e promessa.

Chapter 21Capítulo 21

Decree and Second CausesDecreto e causas segundas

Now the hardest part, the one the previous chapter's objection really wants to discuss. It isn't whether my body is accepted. It's whether I'm allowed to touch it.

Agora a parte mais difícil, a que a objeção do capítulo anterior realmente quer discutir. Não é se o meu corpo é aceito. É se eu posso mexer nele.

Throughout this text, I've held two ideas that seem to fight each other. The first: the body decided first, and I chose nothing of what it did. The second: I decide alongside it, and in January I sign. To someone outside the Reformed tradition, that can sound like a contradiction. To someone inside it, it sounds like Westminster.

Chapter III of the Westminster Confession of Faith says God ordained, from all eternity, whatever comes to pass. And it says, in the same sentence, that this doesn't make God the author of sin, doesn't do violence to the will of the creature, and, in the Confession's own words, "nor is the liberty or contingency of second causes taken away, but rather established." The decree doesn't cancel second causes. It happens through them.

That's what theology calls compatibilism. The decree and the choice don't compete for the same space. God's sovereignty doesn't need me to stand still in order to be fulfilled; it's fulfilled when I decide, too.

In my body, the second causes have names. A gene that didn't win the tug of war. A hormone that didn't erase a uterus. An enzyme that converts. A receptor that reads by half. It was through them that the body decided first, and I chose none of them. And my will, when I sign in January, is one more second cause. It isn't a creature snatching the pen from God's hand. It's the creature using the pen He Himself gave.

No meu corpo, as causas segundas têm nome.

Fig. 9Decree and second causesDecreto e causas segundasAt each fork, choose a side: click or tap a branch, or use ← → and Enter.

An abstract path with five forks and one endpoint, the decree, marked from the start. You choose freely at every fork, and the path is recorded as yours. Walk again: the second path stays beside the first, and both arrive at the same point. The decree and the choice don't compete for the same space.

And here's my position, plainly. Intervening in the body isn't refusing the decree when there's a reason. In my case, the reason is health. And I don't see any church objecting to someone having surgery for a herniated disc. Nobody accuses the person who has their gallbladder removed, gets a pacemaker or corrects a cataract of rebelling against the Creator. Medical intervention, in itself, has never been a sin in any Reformed tradition. The Westminster Larger Catechism goes further: it lists the sober use of medicine among the duties of the sixth commandment, and calls it a sin to neglect the lawful and necessary means of preserving life.

No meu caso, o motivo é saúde.

Calvin had already said this before Westminster. In the Institutes of the Christian Religion, the Institutio Christianae Religionis, he answers those who used providence as an excuse not to take care of themselves: if God set the day of our death, why bother with doctors and medicine? His answer is that the one who set the limits of our life also entrusted us with its care, and gave us the means and remedies to preserve it. If God offers the remedy, it's not for us to despise it. For Calvin, providence almost never appears naked. It comes clothed in the means God uses.

Pra Calvino, a providência quase nunca aparece nua.

The rejoinder to the hernia example is a good one, and it's worth building in full: "Hernia surgery restores a function the body had and lost. A creates a structure your body never had. The analogy doesn't hold."

It holds, but for one part of the surgery, and the two parts have to be separated.

The hernia applies to the . That's the clinical part: gonads that hurt, that have findings, and that carry increased risk. Removing a gonad under those conditions is what's done with any organ that threatens the body. Calling that rebellion would require calling every surgery that removes a diseased organ rebellion.

A hérnia vale pra .

The neovagina has a different argument. It doesn't create from nothing. It completes what the body already started: a genital feminization so strong it changed everything, with the pelvic tilt and the clitorization. My body wasn't sitting still in a male design, waiting to be contradicted. It had already gone, on its own, in the direction the surgery will finish. If the decree is fulfilled through second causes, I know of no Reformed reading that obliges me to stop halfway down a road the body itself opened.

The decree wrote the body. The signature is mine, and it doesn't erase a single line of what was written. It signs off.

O decreto escreveu o corpo. Assina embaixo.

Intersex MattersO intersexo importa

Chapter 22Capítulo 22

  1. 22It Hardly MattersPouco importa

Chapter 22Capítulo 22

It Hardly MattersPouco importa

In the prologue, I said intersex matters precisely where I was told it hardly matters. It's time to come back to that sentence.

"At the end of the day, intersex hardly matters."

"Pouco importa o intersexo no fim do dia."

The person who said it was my endocrinologist, the same one I described, in chapter 15, as someone who knows intersex well. And both things are true at once. That's why the sentence deserves to be read the way he meant it, and not the way it sounds out of context.

What he meant is that, clinically, it hardly matters. A trans person and an intersex person of my type go through almost everything the same way. The difference lies in the details and in the process.

This is the strongest form of the objection, and it comes from someone who knows what he's talking about:

"The doctors are right. The protocol is the same, the surgery is the same, the outcome is the same. Insisting on the label is a matter of identity, not of medicine. To the doctor's office, intersex is a detail."

I don't agree that it's the same. Similar, maybe. But there are things only I go through, like bone remodeling at 26, with the from chapter 10 still open.

And the difference lies in the details and the process, which is where intersex lives. Let me show you where.

Deixa eu mostrar onde.

The first detail nearly knocked me down, literally. In Tokyo, a protocol treated my body as if it were an ordinary male body that had gone off track, and tried to put it back with testosterone. I fainted. What looked like a routine adjustment was, for my body, poison. Nobody in that room was being negligent. They were following the protocol for a body that wasn't mine.

O primeiro detalhe quase me derrubou, literalmente.

The second detail is the antiandrogen. In a trans woman, it's there to block a hormone the body uses well. In mine, testosterone already works by half, and the half that works hurts me. The drug may be the same. The reason for taking it isn't.

The third detail is on the operating table. Inside, my body has structures no protocol designed for another body anticipates, and what to do with them is a question that only exists because I'm intersex. It's the subject of the next chapter.

Ela é o assunto do próximo capítulo.

The fourth detail is risk. Chapter 6 showed that the tumor risk in a gonad like mine isn't anyone's risk. It changes with the diagnosis, with the gonad's position, with what shows up on the ultrasound. The decision to remove it, and when, depends on knowing exactly what body this is.

The fifth detail is the future, and chapter 15 already showed where it comes from: with high , outside estrogen, for me, isn't just affirmation. It may become replacement. If I stop, my body has nowhere to go back to: what follows is .

Fig. 10Generic protocolProtocolo genéricoApply the form field by field: click apply or press Enter. Tab moves to the next field.

A form pre-filled for a standard patient, applied to the same patient in two rounds. The five fields are the chapter's five details: Tokyo, the antiandrogen, the structures on the operating table, gonadal risk, and high FSH and LH. In the first round the protocol reads only the karyotype and each field goes wrong in a different way: the routine adjustment was poison, the antiandrogen's reason is another, the table holds structures it doesn't anticipate, the risk isn't anyone's risk, and stopping estrogen isn't going back, it's hypogonadism. In the second round the form that reads the layers gets the same five fields right. From a distance, it looks like the same road. Up close, every step is different.

Add up the five, and the math is simple. Even where the road looks the same, every step on it depends on knowing which body is walking. In an intersex body, getting a detail wrong isn't a detail error. In Tokyo, it was a whole day in bed.

And here my own position on labels helps close the account. In chapter 14 I said these labels are only good for something in a medical context. Well: the doctor's office is the place where the intersex label makes sense. Outside it, I'm a woman, no suffix. Inside it, the suffix is what protects me.

E aqui a minha própria posição sobre rótulos ajuda a fechar a conta. No capítulo 14 eu disse que esses rótulos só servem pra alguma coisa no contexto médico. Pois bem: o consultório é o lugar onde o rótulo intersexo faz sentido. Fora dele, eu sou mulher, sem sufixo. Dentro dele, o sufixo é o que me protege.

My care today is at NuMA, no longer at Hospital das Clínicas, São Paulo's big university hospital. One word says who I am. The other says how to take care of me.

From a distance, it looks like the same road. Up close, every step is different.

De longe, parece o mesmo caminho. De perto, cada passo é outro.

Chapter 23Capítulo 23

The Uterus I Already HaveO útero que eu já tenho

In chapter 2, I said menstruating is proof that my uterus isn't some forgotten leftover. An attentive reader will take that sentence and turn it against me, and she has every right to try:

"You called menstruation proof. Well, in January you're having your gonads removed, and the ovarian tissue with them. No ovary, no cycle. You're going to switch off the very proof you used to hold up your text."

She's right about the fact and wrong about the conclusion.

Ela acerta no fato e erra na conclusão.

The fact: the removes the gonads, and with them the ovarian tissue inside the . That tissue is part of the cycle I have today. I don't dispute that.

The conclusion doesn't hold, because the uterus doesn't live in the gonads. It's somewhere else in the body, and it will still be there after January. The surgery touches the gonads. It doesn't erase the uterus.

And the proof was never about the future. It was about the past. Chapter 2 used menstruation to show something about where my body came from, not to promise anything about what comes next. An organ that works is an organ that was there, whole, since before I was born. All it took was someone looking inside. No surgery undoes what the body has already shown. What chapter 2 proved stays proven.

What will happen to that uterus's function after January is a different question. Before the surgery, I'm going to ask the team whether it's possible to preserve and channel the function instead of removing the structure.

That question has a history. In Part 1, I wrote about surgeries done on intersex bodies without anyone asking the person who would live with the result, often to make the body look like the sex chosen on paper. Asking first is the exact opposite. It doesn't guarantee any answer. It guarantees that nothing will be decided about me without me.

I don't have the answer yet. When I do, the decision will be mine, together with whoever operates.

A signature doesn't only say what can be taken out. It also says what has to be asked first.

The CalendarO calendário

Up to here, this text has been about whys. This chapter is about what and when.

In January: the and the , in the same operation. The team was direct with me: we're not opening you up twice. The indication is the pain my gonads cause me. The neovagina goes along, because the feminization of my body has already gone so far that separating the two would mean opening the same place twice to reach the same result.

A indicação é a dor que as gônadas me dão.

And what I want from it, I've already said and I'll say again: to be whole and to feel my body, without settling for half.

Before January: the conversation with the urologist about the question from the previous chapter. Whatever is decided there stays between me and my team.

After the neovagina: the , with the cost to my singing already accepted, for the reasons in chapter 18.

In parallel, the face: the laser continues, facial feminization comes as fine-tuning, and the nose goes on the list, along with any other area the doctor recommends.

And the hormones continue every day until then: estradiol gel, split across the day, and cyproterone, with the dose the team will keep adjusting as January approaches.

Fig. 11TimelineLinha do tempoDrag each piece to its place on the calendar. On a keyboard: Enter picks up, the arrows move, Enter drops. No teclado: Enter pega, as setas movem, Enter solta.

A calendar with three places in order, before January, January and after the neovagina, and two bands that run along, in parallel and every day. The gonadectomy and the neovagina only fit January together, in the same operation: we're not opening you up twice. The Wendler glottoplasty only fits after the neovagina. The face runs in parallel, and the hormones, estradiol gel and cyproterone, every day. The question about preserving the Müllerian function fits no date and stays before January, without one. Off the calendar is what isn't going to happen: no method of stopping the delayed puberty, no dose of testosterone. The dates coming now, I'm the one marking.

There's one thing that doesn't appear on any calendar, and it's a decision too. No method of stopping my delayed puberty. No dose of testosterone: Tokyo's recommendation was to stay away from any source of it. And the testosterone my own body makes, cyproterone holds back. That was decided in Tokyo, on the day my body answered by fainting, and it has no date because it isn't going to happen. A calendar shows what's coming. It doesn't show what was refused, and the refusal was the first signature in this text.

Notice that none of these dates is destiny. They're decisions. Each one has a reason written in this text and a signature I have yet to give. And anyone afraid I'll regret it can look at this calendar and see what it shows: nothing here is impulse. Everything here has order, reason and time.

The body marked the beginning on its own, in November 2025. The dates coming now, I'm the one marking.

The EveVéspera

This text ends before January, on purpose.

I could have waited for the surgery, the recovery, the first appointment afterward. It would have had a rounder ending. But it would have been a text about the result, and this is a text about the signature. And the signature happens on the eve.

So let me gather what was left along the way.

The body decided first. In November 2025, it started on its own something I never asked for, with the force of something that had been waiting for that moment for twenty-five years. Gene, hormone, enzyme, receptor: the second causes did their work without consulting me.

O corpo decidiu primeiro. Gene, hormônio, enzima, receptor: as causas segundas fizeram o trabalho delas sem me consultar.

I decide alongside it. Not against it, not in its place. Alongside. Every intervention ahead has a reason that starts in the body: the pain, the risk, what it already did on its own and what's still left.

Eu decido junto com ele. Não contra, não no lugar dele. Junto.

And intersex matters, precisely in the place where I was told it hardly matters. Outside the doctor's office, I'm just a woman. Inside it, the word intersex is what separates a protocol that cares for me from a protocol that knocks me down.

E o intersexo importa, justamente no lugar onde me disseram que pouco importa.

And if you're intersex and you've made it this far, I owe you something said clearly: this text is not a map. My road runs through hormones and surgery because my body, with the pain and the risk it carries, asked for that. Yours may not ask for anything. It may ask for something else. It may ask for time. The only part of this text that I think applies to everyone is the signature: that nothing be done to your body without your yes, and without you knowing exactly what you're saying yes to.

Part 1 called it violence when it's done to someone who can't sign. Part 2 is what happens when someone finally can.

There's still an open question. In the prologue, I told you about a pain that has a cycle and a name. I don't know whether it will continue after January. The answer depends on a conversation that hasn't happened yet.

And some answers, when they come, will stay between me and my team. Not everything that happens in my body needs to become text, and that choice is a signature too.

And that's okay. Part 3 begins on the other side of January.

E tudo bem. A parte 3 começa do outro lado de janeiro.

Until then, here's what this text came to say. It's not the scalpel that decides. The body decided first, I decide alongside it, and the scalpel only signs off.
